DAY OF VISIT INFORMATION

In-Person Office Visit Check List​​ For Existing Patients​

  • All patients should arrive at least 15 minutes ahead of the scheduled start time.

  • Check-in with the staff at the front desk.

  • Have any new paperwork uploaded to the patient portal before visit or bring a copy with you.

  • All patients should have their device turned on and logged in to the online appointment at least 15 minutes ahead of the scheduled start time.

What is the best way to contact the office? 

Call us at 1-845-876-0300 or send us a message using the patient portal. When reaching out to our office, please do so in one place. If you call and we are unable to answer the phone, please leave a message and wait for us to call you back. If you email us, please wait for us to reply to your email.

What is included with my initial visit and treatment plan? 

An initial visit consists of a 90 minute appointment with the doctor to go over your entire health history and lab work.  He will then create a treatment protocol on what he thinks would be best for you to start doing.

What should I do ​​if I have questions about my appointment?Please hold any questions that the doctor will go over (such as asking for lab results) until your actual appointment. If you have any logistical questions regarding the date, time, what will be covered or need to reschedule, please call us ASAP.


How do I follow up if something new is going on? 
If you have a new symptom or need to change the treatment plan (ie. add a supplement, discontinue a medication due to reaction or side effect, want new testing, etc), you will need a new appointment. Please call us at 1-845-876-0300 or send us a message to book you new appointment. â€‹
